[B] Scope of Service:
(1) Vendor needs to provide Medical Case Management and Outpatient Medical Services
for government authority.
(2) Contract period shall be from date of award through February 28, 2013 and it may extend for one additional year.
(3) Vendor should provide Medical case management services which include face-to-face, phone contact, and any other forms of communication.
(4) Vendor needs to conduct an audit for the most recent fiscal year ended available for onsite verification.
(5) Vendor must comply with all financial and service data requirements.
[C] Work Performance:
Performance of the work will be Offsite. Vendor needs to carry work in their office location.
